Introduction

Chorionic epithelial carcinomaIt is a malignant tumor that originates from the embryonic chorion, including all of the two layers of trophoblast. It often occurs in the uterus, but it is not the only primary site. There is a clear relationship with pregnancy, about 50% of chorionic epithelial cancer occurs after hydatidiform mole; 25% after abortion; 22% after normal delivery; others occur in ectopic pregnancy. This is a rare malignant tumor, and the majority of the patients are women under the age of 35.

Overview

Chorionic epithelial carcinoma

Chorionic epithelial cancer is a malignant tumor that originates from the embryonic chorion, including all of the two layers of trophoblasts. The common clinical symptoms of chorionic epithelial cancer are irregular vaginal bleeding, and the amount of blood is uncertain. Sometimes, temporary amenorrhea may occur first, followed by sudden vaginal bleeding. During the examination, the uterus is enlarged and soft, and the shape is irregular. The vagina has a bloody secretion that is soy and smelly. Systemic symptoms are anemia, weight loss, and even dyscrasia, and infection can have fever. Because of the extremely high degree of malignancy, lung metastasis can occur early, and chest pain, cough, hemoptysis and other symptoms appear. Other metastatic sites are vaginal, vulva, pelvic, liver, brain, etc., and the corresponding symptoms of various organs appear. Once the disease is not treated in time, the patient often dies within 1 year.

Staging

The current stage of use in China is:

Stage I: The lesion is confined to the uterus (no metastasis).

Stage II: The lesion is transferred to the para-uterine tissue and attachment (near metastasis).

IIa: Transfer to the parametrial tissue and accessories.

IIb: Transfer to the vagina.

Stage III: The lesion is transferred to the lung (distant metastasis).

III a: The diameter of a single lesion is <3.0 cm, or the flaky shadow does not exceed one-half of one lung.

IIIb: Lung metastasis exceeds the range of IIIa.

Stage IV: The lesion is transferred to the brain, liver, intestine, kidney, etc.

Cause

Modern medicine believes that the cause of this disease is still unclear and may be related to the following factors:

1. Continuous pregnancy and protein deficiency malnutrition in a short period of time;

2. Close relatives are getting married;

3. Some people see virus particles under an electron microscope, so it is considered to be related to viral infection.

diagnosis

The main basis for the diagnosis of this disease:

After the postpartum or abortion, especially after the hydatidiform mole, vaginal bleeding continues, the uterus is relapsed, the uterus is large and soft, the urine pregnancy test continues to be positive, the symptoms of curettage are not improved, and the chest X-ray has a knot in the lungs. Section shape. Cotton globular or flaky metastatic shadows should be considered for this disease. The determination of chorionic gonadotropin (HCG) has important reference value for the diagnosis of this disease. The curettage can be confirmed by finding the chorionic epithelial cancer cells, but sometimes the tumor can be false negative between the muscle walls.

treatment

The treatment of this disease in modern medicine is mainly chemotherapy. In the absence of metastasis, 90% of patients can be cured, and the remaining 10% of patients have a significant increase in HCG titer, indicating that there are still tumors remaining, requiring total hysterectomy. If the HCG titer falls to normal and then rises, it is an indication of total uterine resection or multiple drugs combined with chemotherapy. If brain metastasis occurs, brain radiation therapy should be added.

The disease is mostly in the category of "ghosts" and "leakage" in the clinical practice of Chinese medicine. For example, the "Certificate of Correction" contains: "There are more than a hundred pieces of meat, there are eyebrows", "There is no delay in March, but it has to be leaked." The motherland medicine believes that the cause of this disease is that the pulse is cold, the air machine is blocked, the blood stasis is stagnant, and the tumor is accumulated. If it is not treated immediately, the cult poison will grow day by day, making it weak and even incurable.
